Have you ever visited us and wondered why we can’t see all of your health and care information? Or queried why you sometimes need to answer the same questions about your medications, allergies or treatment each time you visit your local GP or hospital?
The simple answer is we all use different computer systems to record your details. However, that’s now changing with the introduction of the Coventry and Warwickshire Integrated Care Record (ICR).
The new Integrated Care Record for #Coventry and #Warwickshire will improve patient health and care by:
- Offering better, faster treatment
- Allowing clinicians to see your medications – making treatment safer
- Not having to repeat your details every time you need care
The ICR is bringing together the various electronic records of a person’s care and presenting it in a structured and easy-to-read format for health and care professionals to use.

Patients right to object: For any patients who wish to object to these records being made available, please ask the patient to complete the following form. Please note the postal form must be used if the objection is completed on behalf of someone else ( provided on the same link).

The Integrated Care Record allows health and care professionals involved in your care to view your records to help them understand your needs and make the best decisions with you, and for you. Information we hold about you will be available, to read only, to other Health and care professionals in Coventry and Warwickshire, Birmingham and Solihull, and Herefordshire and Worcestershire when they are involved in your health or social care.
